Certified that I have gone through the Mess Rules and undertake to abide by the rules and regulation as that may be made by the administration from time to time. In the event of any default. I realize that allotment will be cancelled without any Notice.

The Defense saving certificate and last pay will be given after physical possession of the room is hindered to concerned authority (Applicable to House Officers & TRMOs).

The Defense saving certificate and certificate will be given after physical possession of the room to concerned authority (Applicable to postgraduate Student).

No objection certificate will be given after physical after physical possession of the room to concerned authority (Applicable to Medical Officer).

I will reside with family permanently and physical. The family room will be cancelled and disciplinary action will be taken the family not found (Applicable to medical Officer).
