MBBS Affidavit – MEDICAL ADDMISSION FOR COLLEGE 2025

AFFIDAVIT

I ________________________________________    /o, ______________________________ adult R/O________________________________________  a candidate for admission in 1st year MBBS for the academic session 2024-25 do hereby state on solemn affirmation/oath as under:

1. That I am a holder of PRC of District/Division _____________________ which was issued based on the domicile of District/ Division_____________________ issued to me/ my father/ mother, and I have no other PRC /Domicile of any other district of Sindh.
2. That if I am allowed admission in MBBS of the merit seat of District/Division_____________________ on the basis of Domicile/ PRC. I shall not seek admission in any other public sector/ private sector medical or dental college/ medical university or any other degree college/ university of the country against any seat of another district.
3. That all documents submitted by me along-with application form are genuine and correct and no false document has been submitted. If any document/ information is found to be false, incorrect or otherwise I know that my admission is liable to be cancelled.
4. That after my final selection/ provisional admission on open/ finance seat in MBBS course, I shall complete the course of five (05) year MBBS, In case I leave the (____________________________________________) before completion of the course, I shall be liable to pay the full five-year fee payable through pay order/ demand draft for wasting the precious seat of MBBS, else I will not be entitled to issuance of any certificate from the institution.
5. No migration either external or internal in any circumstances is allowed during the whole MBBS course in This Institute; however, in case I leave the (__________________________________________) before completion of the
6. course, I shall be liable to pay the Full five-year fee (merit/self finance).
7. That I also undertake not to involve myself in any kind of political activity throughout my tenure in this institution as a student and incase I am found involved in such activities, the authority of the institution shall be at liberty to expel/ rusticate me and to cancel my admission.
8. That I also state that I have gone through all the rules,, terms and conditions of the admission, studies, expulsion and termination and solemnly declare that I will be bound to abide with or liable to mentioned penalty in case of breach of any such rule.
9. That whatever is stated above is true and correct to the best of my knowledge and belief.
10. I am fully aware that selected candidate should submit fee within 3 working days after confirmation call of admission otherwise admission will be deemed cancel and next candidate on merit will be offered the seat, which will not be challengeable in any court of law, and Fee deposit is not final evidence of final admission and could be return if either candidate documents found fake/ duplicated or any other dispute regarding merit/domicile verification between candidates.
