1. Student’s Core Undertaking

• Adherence to Rules: The student solemnly declares to have read all the Hostel Rules and Regulations and undertakes to abide by all such rules/policies established by the Shaheed Mohtarma Benazir Bhutto Medical University (SMBBMU).
• Consequences of Violation: Failing to comply with the rules shall result in severe penalties, including censure, fine, disciplinary action, eviction from the hostel, or any other action suggested by the university/Hostel Administration.
• Authority: The declaration is formally addressed to, and the student is accountable to, the Hostel Provost.

2. Parent/Guardian/Surety Core Undertaking

• Responsibility for Conduct: The parent/guardian takes explicit responsibility for the student’s conduct and ensures that the student will follow the norms of the SMBB Medical University Code of Conduct while both inside and outside the Hostel.
• Financial Surety: The parent/guardian assumes responsibility for the payment of all hostel dues and any penalties imposed upon the student under the university’s rules and regulations.
• Availability on Call: The parent/guardian guarantees they will be available on call and promises to visit and take care of the ward, as and when required by the Hostel/University Administration.
• Accountability for Incidents: The parent/guardian takes personal and legal responsibility for “incident, whatever” involving their ward.

3. Formal Documentation Requirements

• The student must provide their full Name, Father/Mother Name, University Roll No., Batch, CNIC No., Name of the Institute, and Programme.
• The parent/guardian must provide their full Name, Address, CNIC No., and two Contact Numbers.
• The declaration requires details (Name, Address, CNIC No.) and signatures of two independent Witnesses to act as further surety.
