DUET Admission Affidavit & Undertaking Form

AFFIDAVIT

I ___________________________________   /o ____________________________, Muslim, adult and resident of ______________________________________________________________ ___________________________________, do hereby declare and state one solemn affirmation as under:-

1. That I am deponent of this affidavit and well conversant with the facts therein.
2. That I have sought admission in DAWOOD UNIVERSITY OF ENGINEERING AND TECHNOLOY, (DUET) KARACHI AFTER FULFILLING THE ELIGIBILITY CONDITIONS AS PRESCRIBED IN  THE DUET PROPECTUS FOR ADMISSION.
3. That I have not secured admission in any other educational institute in Pakistan.
4. That, in case I am admitted and join Dawood University of Engineering and Technology, Karachi, I shall not join any other Engineering Institute and continue to stay in Dawood University of Engineering and Technology, Karachi.
5. That in case a valid and genuine claim is registered by a candidate on merit with higher percentage that, I shall vacate the seat.
6. That I have submitted correct and genuine documents with my application for admission, I hereby undertake that in case, after verification, the same are found incorrect, forged or tempered at any stage after my admission I shall stand liable to be expelled from the university summarily.
7. That I shall be abiding by all rules and regulations of the university enforced from time to time.
8. That, I shall attend the class punctually and regularly and I shall keep up the attendance upto minimum 75% of the batch as required under the rules in every respect.  Also I understand that in case my attendance fail short of 75% I shall not be allowed to appear in the subsequent examination.
9. That, I shall stick to my studies and shall not involve into any political or subversive activities which may harm the academic atmosphere of the university or give bad name to the institution.
10. That, I shall duly respect my seniors, the administration and above all the teaching faculty at all times. In case I am found misbehaving or guilty of any misconduct I shall be liable to disciplinary action, penalty, punishment as may be imposed by the competent authority.
11. That I understand and agree that the university authorities are not responsible for providing hostel accommodation to me.
12. That, I understand the Vice Chancellor of the university is competent to reject my application and to cancel or withdraw my admission without assigning any reason.
13. That WHATEVER is stated above is true and correct to the best of my knowledge and belief.

DEPONENT

UNDERTAKING

I ____________________________    /o ____________________________, HEREBY UNDERTAKE, IF ADMITTED, TO ABIDE BY THE FOLLOWING:

1. That I shall not indulge in national-regional-ethic politics.
2. That in case I indulge in any of politics, violating the above undertaking, I understand that I will be expelled from the University without any further notice.
