 AFFIDAVIT

I __________________________________ S/D/o ___________________________, holding C.N.I.C No. ____________________, resident at House No. _________________ _____________________________________, hereby Solemnly affirm and declare as under:

1. That I am the deponent of this affidavit and fully conversant with facts mentioned herein.
2. That I have lost my “O” level Equivalence Certificate Serial No. ________________ issued by IBCC (Inter Board Committee of Chairman) Karachi.
3. That inspite of my best study effort I could not trace the same.
4. That nothing has been concealed and mis-represented.
5. That I further declare if the original Equivalence Certificate is found it shall not be used and returned to the issuing authority.
6. That this affidavit is submitting for the issuance of Duplicate Equivalence Certificate of the same.

Whatsoever has been stated above is true and correct to the best of my knowledge and belief.
